Dance and Movement Class Liability Waiver and Release of Liability

Participant Information

Assumption of Risk

I understand that participation in dance, movement, fitness, stretching, mindfulness, somatic practices, and other related activities (collectively, the “Activities”) involves inherent risks. These risks may include, but are not limited to, muscle strains, sprains, falls, collisions, dizziness, fatigue, aggravation of pre-existing conditions, broken bones, or other injuries that may result from participation.

I acknowledge that I am voluntarily participating in these Activities with full knowledge of the potential risks involved. I certify that I am physically able to participate and have consulted a healthcare provider if I have any medical condition or concern that could affect my participation.

Medical Responsibility

I understand that it is my responsibility to:

• Participate only within my personal physical limits.

• Inform the instructor of any injury, illness, pregnancy, or medical condition that may affect my participation.

• Stop participating immediately if I experience pain, dizziness, shortness of breath, or any other concerning symptoms.

I understand that SEE WHY STUDIOS and its instructors do not provide medical advice or medical treatment.

Release of Liability

In consideration of being permitted to participate in the Activities, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless SEE WHY STUDIOS, its owners, officers, directors, employees, contractors, instructors, volunteers, agents, and affiliates from any and all claims, liabilities, demands, causes of action, damages, losses, or expenses arising out of or related to my participation in the Activities, including personal injury, property damage, or death, except where prohibited by law or resulting from gross negligence or willful misconduct.

Emergency Medical Care

In the event of an emergency, I authorize SEE WHY STUDIOS to seek emergency medical treatment on my behalf if I am unable to do so. I understand that I am solely responsible for any medical expenses incurred.

Personal Property

I understand that I am responsible for my personal belongings while attending classes. SEE WHY STUDIOS is not responsible for lost, stolen, or damaged personal property.

Acknowledgment

I have carefully read this Liability Waiver and Release of Liability. I understand its contents, understand that I am giving up certain legal rights by signing it, and sign it voluntarily without coercion.

If Participant is Under 18:

I am the parent or legal guardian of the above-named participant. I have read and understand this waiver and consent to the participant’s involvement in the Activities under the terms stated above.
